If you want to order patient documents from Finla Occupational Health, you must fill an ordering form (you can download the form from here). The form must be delivered personally to the closest health station and you must show your valid ID when ordering the documents.
You can prove your identity with
- a passport
- an identity card that has a picture on it
- a driving license
- a KELA-card that has a picture on it
- an Alien’s passport
When placing an order, you may choose whether the documents are sent to your home address or whether you get them personally from the health station. An estimated time for the delivery is about 1-2 weeks.
Please note that you must prove your identity also when you get the documents personally from the health station.
